Case Study in Implementation Science: Co-Designing National Guidelines for Perennial Malaria Chemoprevention in the Republic of Congo

Background

Malaria continues to pose a significant public health challenge in the Republic of Congo, contributing to high morbidity and mortality rates, particularly among vulnerable populations such as children under five and pregnant women. In response to this pressing issue, the Ministry of Health, through its National Malaria Control Programme (NMCP), recognized the necessity for a comprehensive approach to malaria prevention. This led to an initiative, funded by Expertise France, aimed at developing national guidelines for perennial malaria chemoprevention (PMC)in French. This case study outlines the collaborative process employed to co-design these guidelines and their associated tools, emphasizing extensive stakeholder engagement and preparation.

Objectives

The primary objectives of the initiative were to:

  1. Develop validated national guidelines for perennial malaria chemoprevention.
  2. Create data collection tools and implementation plans at both national and subnational levels.
  3. Establish budgets and communication plans to facilitate the effective dissemination and implementation of the guidelines.

 

Methodology

The project followed a participatory approach, emphasizing co-design principles to ensure that the guidelines were contextually relevant and widely accepted by stakeholders. The process involved several key steps:

  1. Preparation and Dialogue: Over several months, the project team engaged in preparatory activities that included extensive dialogue with various stakeholders. This phase involved identifying key participants from government entities, non-governmental organizations, healthcare providers, and community representatives to ensure a diverse range of perspectives were included.
  2. Validation Workshop: A three-day validation workshop was organized in Brazzaville from July 24 to 26, 2025, attended by 30 stakeholders representing different sectors and levels of the health system. The workshop aimed to present the draft guidelines, solicit feedback, and collaboratively refine the content. Participants engaged in group discussions, breakout sessions, and presentations to share their insights and experiences related to malaria prevention.
  3. Outputs Development: The workshop resulted in several critical outputs:

– Validated Guidelines: The primary output was a set of nationally validated guidelines for perennial malaria chemoprevention, reflecting the collective input of stakeholders.

– Data Collection Tools: The team developed standardized tools for data collection to monitor the implementation and impact of the chemoprevention strategy.

– Implementation Plans and Budgets: Detailed implementation plans were created for both national and subnational levels, outlining necessary resources, timelines, and responsibilities. Associated budgets were also formulated to ensure financial feasibility.

– Communication Plans and Tools: Comprehensive communication strategies and materials were developed to facilitate the dissemination of the guidelines and ensure stakeholders were informed and engaged throughout the implementation process.

Results

The co-design process led to the successful development of a robust framework for perennial malaria chemoprevention in the Republic of Congo. The validated guidelines provided a clear and actionable roadmap for the NMCP, ensuring consistency in the approach to malaria prevention across the country. The involvement of diverse stakeholders not only enhanced the quality of the guidelines but also fostered ownership and commitment to the implementation process.

Conclusion

This case study illustrates the effectiveness of a collaborative, co-design approach in developing national guidelines for perennial malaria chemoprevention in the Republic of Congo. By actively engaging stakeholders throughout the process, the NMCP was able to create guidelines that are not only scientifically sound but also culturally and contextually relevant. The outputs from this initiative—including validated guidelines, data collection tools, implementation plans, and communication strategies—provide a comprehensive framework for addressing malaria prevention effectively, ultimately contributing to improved health outcomes in the region.

Recommendations

For future initiatives, it is recommended to:

– Continue engaging stakeholders beyond the guideline development phase to ensure sustained commitment and adaptation to emerging challenges.

– Monitor and evaluate the implementation of the guidelines regularly to assess their impact and make necessary adjustments.

– Foster partnerships with local communities to enhance ownership and facilitate grassroots-level implementation of malaria prevention strategies.

This case study serves as a model for future interventions aimed at addressing public health challenges through collaborative and evidence-based approaches.
